Make a Referral

Thrive welcomes referrals from LACs, allied health professionals, plan managers and other community partners. This page outlines who can refer, what information helps, and how the process works.

Who can refer

Referrals are welcome from anyone supporting a participant

There is no single referral pathway. Thrive accepts referrals from a wide range of sources, including self-referrals from participants and families.

  • Local Area Coordinators (LACs)
  • NDIS Early Childhood Partners
  • Allied health professionals
  • Plan managers
  • Community organisations
  • Families, carers and nominees
  • Participants referring themselves
  • Other support coordinators (for transition)

Helpful information to have ready

You don't need to have everything — a brief conversation is enough to get started. The following helps us respond quickly:

  • Participant's name and contact details
  • NDIS participant number (if available)
  • Plan management type (plan-managed or self-managed)
  • Brief overview of the participant's situation and support needs
  • Any urgency or time-sensitive factors
  • Your contact details for follow-up

Eligibility note: Thrive supports plan-managed and self-managed NDIS participants who have Support Coordination or Specialist Support Coordination funded in their current plan.

The process

How a referral works

Make contact

Call or email Thrive to discuss the participant's situation and check availability. No formal referral form is required to start a conversation.

Confirm eligibility

We'll confirm the participant has Support Coordination or Specialist Support Coordination funded in their NDIS plan and is plan-managed or self-managed.

Participant consent

The participant (or their nominee) provides consent for Thrive to be engaged as their support coordinator. We can assist with this step.

04

Service agreement

We complete a service agreement with the participant outlining the scope of support, how we work together, and the relevant NDIS funding details.
